늑연골 이식을 이용한 악관절 재건술 : 증례보고

Abstract
결론
저자 등은 악관절 강직 및 안모 비대칭을 주소로 본원에 내원한 3명의 환자에게 늑연골
이식을 이용한 하악골 재건술을 시행하여 다음과 같은 결론을 얻었다.
1. 악관절 강직 및 수지상 이식재와 Kirschner wire의 파절로 인하여 악기능이 제한된 환자
에서, 늑연골 이식을 이용하여 적절한 안면 고경의 신장 및 악기능을 재건할 수 있었다.
2. 성장이 끝난 성인에서 이식재의 부가적인 성장없이 적절한 안모 대칭을 이를 수 있었다.
3. 적절한 구치부 고경의 유지를 통하여 이식골의 흡수없이 안면 고경 및 악기능을 유지할
수 있었다
4. 과도한 개폐구 운동으로 인해 이식재의 파절이 야기될 수 있으며, 부가적인 외과적 처치
없이 지속적인 기능적 물리 치료만으로 적절히 치유될 수 있었으나, 이식골의 적절한 안정
성을 유지하기 위하여 이식 골편은 관절와에 대해 세로로 이식하고 악간 고정은 8주 정도
시행하는 것이 좋을 것으로 사료되며, 악간 고정 제거후 과도한 개폐구 운동은 이식재의 파
절을 야기할 수 있으므로 골개조가 끝나는 6개월까지는 무리한 힘을 주지 않는 것이 좋을
것으로 사료되었다.

The aim of this article was to examine the usefulness of costochondral graft in the
temporomandibular joint reconstruction.
We performed costochondral grafting in the mandibular reconstruction including
condyle replacement in 3 patients, which 2 of ankylosed cases and 1 destructed case.
In one case, unexpected fracture of costochondral junction has developed at
post-operative 2 months, but, normal mouth opening and facial appearance have been
acheived by continuing exercise.
These patients showed maximum mouth opening of 35-40 mm and no restriction of
lateral side movement. They have been improved esthetics and function.
We proposed that the costochondral graft is one of the useful method for functional
reconstruction of defected mandibular condyle.
